1. Apple Health (Medications)

Built into the Health app since iOS 16. Add a medication by name (or by scanning the label in the U.S.), set a schedule (daily, specific days, every few days, cyclical or as needed) and log each dose as taken or skipped. Follow-up reminders fire if a dose isn't logged after 30 minutes, and Critical Alerts can sound during Focus. In the U.S. it checks interactions between your medications. You can share your list with a loved one or export a PDF.

Missing: pill counts and refill alerts.

2. DoseMed

A medication reminder that works without an account. Reminders arrive with Taken, Snooze and Skip buttons, Critical Alerts are optional, and each dose you take is subtracted from your pill count so you get a warning before you run out. It includes 9,900+ FDA labels from DailyMed, an AI assistant that knows your list, Home Screen widgets and CSV export. Free with every feature; Premium ($0.99/week, $2.99/month or $9.99/year) removes the limits of 1 active reminder and 3 AI messages.

Missing: Apple Watch, caregiver alerts, cyclical or every-few-days schedules.

3. Medisafe

The most established dedicated app (4.7 stars from 101K U.S. ratings). Refill reminders, drug interaction warnings, Medfriends who can be alerted about missed doses, dependents, health trackers and an Apple Watch app. Since January 1, 2026 it requires a subscription outside the U.S. ($4.99/month or $39.99/year).

Missing: a free plan outside the U.S.

4. MyTherapy

Free and ad-supported, in 32 languages. Reminders, refill alerts, a symptom and pain diary, 20+ health measurements and reports for doctor visits. An ad-free subscription and a lifetime Plus purchase are available.

Missing: ad-free use without paying; its App Store privacy label lists identifiers used to track you.

How to choose

  1. Do you need someone else to know? If a caregiver must be alerted, pick Medisafe. If they only need to see the list, Apple Health sharing is enough.
  2. Do you run out of pills? Only the dedicated apps track supply. DoseMed, Medisafe and MyTherapy all have refill reminders.
  3. How much do you care about privacy? DoseMed needs no account and keeps data on the device. Apple Health keeps data on your devices and in iCloud.
  4. Is your schedule unusual? Cyclical packs and every-few-days doses are easiest in Apple Health.
